Questions and answers

Entering the patient's date of birth and attaching a plan report are required in order to return a case to OPM. If you have additional documentation to submit to OPM please add it to the Supporting Documents section of the case page. 

When you have entered and uploaded all the required information, press the "Return to OPM" button at the bottom of the page. The OPM Customer Service Representative or Claims Examiner that requested the reconsideration receives an email notification automatically when you return a case with a Reconsideration File.
