If you are enrolled for family life insurance (Option C), and a covered member of your family dies, you can contact us in one of three ways:

    • Send us email at retire@opm.gov
    • Write to us at:

      Boyers, PA 16017-0045.

Download the claim form (FE-6 DEP), which includes instructions, or contact OPM to request a form. Note that The Office of Federal Employees Group Life Insurance (OFEGLI) requires an original copy of the death certificate.
